The treatment of neck and upper back pain begins with identifying a list of possible diagnoses. Multiple structures can be affected when pain is present in this area – a thorough history and physical examination assists in deciphering what is potentially causing the discomfort and/or pain. Biomechanically, it is important to consider the neck and upper back together along with other distant, seemingly unrelated structures including the hips, pelvis, low back and upper extremities.
This presentation is particularly common amongst desk workers and can commonly present alongside headaches and/or upper extremity pain or discomfort.
This presentation generally responds very well to manual therapy in conjunction with simple home (or work) exercises to manage symptoms. It is also important to consider postural compensations in such presentations. Postural strength exercises are often prescribed in these cases to ensure long lasting change.
